INTAKE FORMS
All Intake Forms and Practice Agreements will be sent to you via email through our scheduling system, Kalix. To request these forms, please send us an email! Once the forms are completed, we will be in touch regarding availablity for an initial appointment. The forms also contain more information about policies, insurance, and rates.

INITIAL APPOINTMENT
The Initial Appointment is designed to get to know you better, understand your background, and allow you to ask any questions you may have about the nutrition therapy process. The Initial Appointment is about 50-60 minutes. In this appointment, we will establish treatment goals and connect you to any supplemental care you may need.
All virtual appointments will take place through the doxy platform. Click here to join Megan’s doxy waiting room.

FOLLOW-UP APPOINTMENTS
Nutrition counseling is often a long-term process. We typically work with clients anywhere from a couple months to even years! Healing from an eating disorder requires regular therapy and monitoring, as well as long-term support to prevent relapse. Follow-up appointments are about 50-minutes and will take place as frequently as needed. Frequency of appointments will be determined in the Initial Appointment.

INSURANCE & RATES
We are in-network with multiple insurance plans throughout the state of Kentucky. For those out of state, or not covered under the below plans, we are happy to offer you a Super Bill for potential out-of-network reimbursement alongside our private pay rate.
Our practice is currently in-network with Humana, Anthem BCBS of Kentucky, and United Healthcare. If you would like to use your health insurance to cover your appointments, please be sure to let us know by providing a copy of your insurance card in the New Client Forms. Coverage is dependent on your specific insurance plan. It is important to check your individual plan information to learn if you have covered services.
For those who have a plan that we are not in-network with:
All clients with out-of-network benefits will receive a Super Bill following each session. The Super Bill is essentially a receipt of services that you will submit to your insurance company for reimbursement back to you, if you have out-of-network nutrition counseling benefits. Many plans will have some coverage, even if we are not in-network. Please contact us for more information on how to learn about your inidividual out-of-network benefits.
Rates for out-of-network or private pay clients:
Initial session: $130 / 50-60 minute session
Follow-up sessions: $125 / 50 minute session
Team talks: $375 / presentation
Full policies are provided in the Practice Agreements Intake Form.
Payment is due at the time of service, unless other arrangements with the practice have been made. We accept HSA/FSA cards, checks, and most major credit and debit cards. If you are utilizing insurance to cover our sessions, you will not be responsible for any fees up front, but will be required to keep a credit card on file for any potential balances not covered by insurance.

I don't have an eating disorder - can I still work with you?
Absolutely. You do not have to meet diagnostic criteria for an eating disorder to still be deserving of care. We work with anyone who has a nutrition-related concern that interferes with a happy, healthy relationship with food.
What are your business hours?
We offer appointments Monday through Thursday from 9am to 5pm at the start of each hour. We also offer meal support sessions around meal times if part of your treatment plan.
Will you work with my treatment team?
Yes! Best practice for eating disorder care states that treatment is most effective when a client has a full team around them. This typically includes a doctor, psychiatrist, therapist, and dietitian. Seeking care without a full team will often lead to poorer outcomes.
Why should I see a dietitian?
Dietitians are uniquely trained to help you heal your relationship with food. We not only help you determine what to eat, but also challenge how and why you think the way you do about food and your body. A dietitian can help you find a pattern of eating that is truly nourishing, both physically and mentally.
Who do you work with?
We accept clients ages 13 and over. All clients under the age of 13 will be accepted on a case by case basis. All races, ethnicities, and gender identities are welcome here.
